Annual Salary: $130k – $170k (varies depending on years of experience)
We are seeking a Primary Care Nurse Practitioner to join a dynamic and mission-driven primary care practice serving a diverse urban community. The clinician will provide comprehensive, patient-centered care as part of a multidisciplinary healthcare team. This position focuses on promoting wellness, managing chronic conditions, and improving outcomes through proactive, coordinated care.
Key Responsibilities
- Deliver high-quality primary and preventive care to a culturally diverse adult and family population.
- Conduct comprehensive assessments, manage acute and chronic conditions, and support health promotion and disease prevention initiatives.
- Collaborate closely with physicians, nurses, and allied health professionals to ensure integrated, team-based care.
- Engage in care coordination, population health initiatives, and quality improvement projects that support clinical excellence and sustainability.
- Participate in the development and implementation of clinical policies and best practices within the primary care team.
- Contribute to the center’s patient-centered medical home model, emphasizing access, communication, and continuity of care.
- Maintain accurate and timely documentation in the electronic health record in compliance with HIPAA and regulatory standards.
- Participate in peer reviews, staff training, and committee meetings to advance patient care initiatives and quality metrics.
- Provide culturally sensitive care, ensuring communication and treatment approaches are accessible to Spanish-speaking patients.
Qualifications
- Master’s degree or higher from an accredited Nurse Practitioner or Family Nurse Practitioner program.
- Active New York State NP license and board certification in Family, Adult, or Primary Care (FNP, ANP, AGNP, or ENP).
- Fluency in Spanish and English (medical or conversational proficiency required).
- Prior experience in family medicine, internal medicine, or community-based primary care preferred.
- Strong communication, teamwork, and organizational skills with a commitment to equitable, patient-centered care.
