We are seeking a Nurse Practitioner to conduct Evaluation & Management (E&M) visits for Medicare patients in their homes. This role involves direct patient care, travel between patient residences, and timely documentation of visits.
Key Requirements:
- Active, unrestricted NP license (FNP or Adult NP preferred)
- Board certification as an NP in relevant specialty
- Experience with E&M visits and/or home health strongly preferred
- Reliable transportation and valid driver’s license
- Strong clinical assessment and documentation skills
- Ability to work independently and manage daily schedules
- Comfortable using EMR systems for visit documentation
