Annual Salary: $130k-$180k (varies depending on years of experience)

We’re seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join a dynamic primary care team. This role focuses on delivering high-quality, patient-centered care to a diverse population, with an emphasis on collaboration, education, and continuous improvement.

Key Responsibilities:

  • Conduct thorough patient assessments, including history-taking and physical exams
  • Develop and implement individualized treatment plans based on clinical evaluations
  • Prescribe medications and therapies in accordance with best practices and regulatory guidelines
  • Order and interpret diagnostic tests to support clinical decision-making
  • Educate patients about their health conditions in clear, accessible language
  • Maintain accurate and timely documentation in the electronic health record system
  • Coordinate care by making appropriate referrals and consulting with supervising providers when needed
  • Engage in regular team meetings and contribute to quality improvement initiatives
  • Uphold patient confidentiality and adhere to ethical standards of care
  • Collaborate with medical and administrative staff to ensure seamless care delivery
  • Utilize standing orders and support staff effectively to streamline patient care
  • Demonstrate openness to feedback and a commitment to professional growth

Requirements:

  • Completion of an accredited Nurse Practitioner or Physician Assistant program
  • Minimum of 3 years of clinical experience in a primary care setting
  • Experience working with underserved or culturally diverse populations is highly valued
  • Strong interpersonal skills and a proven ability to work collaboratively in a team environment
