Flexible Monday–Friday schedule | No call | 100% outpatient

About the Role

A well-established, physician-led outpatient group on Long Island is seeking a skilled Internal Medicine Nurse Practitioner to join their team. The practice is known for clinical excellence, patient satisfaction, and strong collaboration across disciplines. This is a fully outpatient role with no on-call responsibilities and a flexible schedule.

Key Responsibilities

  • Deliver high-quality patient care in an outpatient primary care setting, managing preventive, acute, and chronic conditions

  • Refer patients to appropriate specialty services as needed

  • Educate patients about diagnoses, treatment plans, and preventive health strategies

  • Maintain accurate and thorough documentation in the EMR

  • Collaborate with physicians, specialists, and care teams to ensure seamless patient care

  • Provide clinical oversight and mentorship to nursing and support staff

  • Ensure compliance with HIPAA, NYS DOH, CMS, and payer guidelines

  • Participate in population health initiatives and quality improvement projects

  • Support the growth and management of a sustainable patient panel

Qualifications

  • BSN and MSN from an accredited nursing program

  • NYS RN & NP license, BLS, and Board Certification in Internal Medicine

  • Strong clinical judgment, communication, and interpersonal skills

  • Team-oriented, patient-centered approach to care

  • Familiarity with EMR systems and value-based care metrics a plus

  • Entrepreneurial mindset and interest in growing a thriving patient panel
