About the Role
A well-established, physician-led outpatient group on Long Island is seeking a skilled Internal Medicine Nurse Practitioner to join their team. The practice is known for clinical excellence, patient satisfaction, and strong collaboration across disciplines. This is a fully outpatient role with no on-call responsibilities and a flexible schedule.
Key Responsibilities

Deliver high-quality patient care in an outpatient primary care setting, managing preventive, acute, and chronic conditions

Refer patients to appropriate specialty services as needed

Educate patients about diagnoses, treatment plans, and preventive health strategies

Maintain accurate and thorough documentation in the EMR

Collaborate with physicians, specialists, and care teams to ensure seamless patient care

Provide clinical oversight and mentorship to nursing and support staff

Ensure compliance with HIPAA, NYS DOH, CMS, and payer guidelines

Participate in population health initiatives and quality improvement projects

Support the growth and management of a sustainable patient panel
Qualifications

BSN and MSN from an accredited nursing program

NYS RN & NP license, BLS, and Board Certification in Internal Medicine

Strong clinical judgment, communication, and interpersonal skills

Team-oriented, patient-centered approach to care

Familiarity with EMR systems and value-based care metrics a plus
