Job description The APP provides direct clinical services to patients with substance use and co-occurring psychiatric and medical conditions. The APP Responsibilities include assessing patients, collaboratively developing and implementing treatment plans, prescribing medications, monitoring and evaluating treatment progress and collaborating with the Chief Medical Officer and multidisciplinary team. Job Responsibilities

Conducts patient assessments and provides therapeutic interventions including the provision of ongoing medication management services in collaboration with the Assistant Medical Director and Medical Director.
Provides medical health services, education, counseling and emotional support to patients
Develops and implements patient management plans and assists in provision of continuity of care
Orders, reviews, interprets and explains laboratory results to patients
Provides patient education regarding medications, risks, benefits and reasonable outcome expectations
Maintains timely documentation of services including weekly progress notes, medication informed consent, discharge summaries and other required information in compliance with organization, State and federal standards
Provides primary medical care services as needed, in accordance with statutes, regulations and protocols regulating the profession
Maintains confidentiality of patient information in compliance with HIPAA and 42 CFR Part 2

Qualifications

Master of Science from an accredited college or university.
PA-C or FNP-BC with active Vermont state license
Experience in behavioral health, addiction medicine, medically supervised withdrawal management or primary care preferred
Active DEA License

Working Hours

Part-time or Full-time availability
8-hour days

Skills

Knowledge of substance use disorders
Knowledge of the recovery field
Ability to maintain ethical and professional boundaries with clients and staff members
Knowledge of the process and symptoms of acute withdrawal from substances
Proficiency in Microsoft Office Applications including Excel, PowerPoint and Word
