Nurse Practitioner (FNP/AGNP) - San Jose, CA

Vivo HealthStaff is recruiting for a Nurse Practitioner in San Jose, California. This is a full-time permanent position offering comprehensive benefits, CME support, and advancement opportunities in a growing care model.

We are seeking a Vietnamese, Mandarin or Cantonese-speaking Nurse Practitioner to join a clinic-based team serving a geriatric and medically complex population. The ideal candidate will have at least one year of primary care or geriatric experience and be passionate about delivering patient-centered care within a collaborative team environment.

Position Details:

• Job Title: Clinic Nurse Practitioner

• Location: San Jose, California

• Schedule: Full-Time, Monday to Friday

Key Responsibilities:

• Provide comprehensive primary care services to aging and medically complex patients

• Perform routine exams, chronic disease management, and acute care visits

• Collaborate with physicians and interdisciplinary team members to develop individualized care plans

• Educate patients and families about treatment plans, medications, and wellness strategies

• Maintain accurate documentation in the EMR system

• Participate in clinic quality improvement and care coordination initiatives

Requirements:

• Licensed as a Nurse Practitioner in the state of California

• Active DEA registration

• Fluent in Mandarin or Cantonese or Vietnamese (required)

• At least one (1) year of experience in a primary care or geriatric setting

• Strong clinical decision-making and communication skills
