We are seeking a dedicated and compassionate Family Medicine Nurse Practitioner to join our team. The ideal candidate will provide high-quality medical care to patients of all ages, focusing on preventive care, chronic disease management, and acute care. This position offers a balanced work schedule with a low patient volume, allowing for more personalized patient interactions and a better work-life balance.
Job Responsibilities
Provide preventive care, including immunizations, screenings, and health education.
Manage chronic conditions such as diabetes, hypertension, and asthma.
Address acute medical issues and provide appropriate treatment.
Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care.
Maintain accurate and up-to-date patient records.
Participate in quality improvement initiatives and continuing education.
Qualifications
Certification as a Family Nurse Practitioner (FNP).
Valid nurse practitioner license to practice in NY.
Strong communication and interpersonal skills.
Commitment to providing compassionate and patient-centered care.
Ability to work effectively in a team-oriented environment.
