Job Description
Provide care appropriate to the patient population served, including routine health maintenance, management of chronic conditions, and management of acute illness for patients. The FQHC is located in the heart of Boston.
About the position:
- Full-time hours may include evening shifts and Saturday rotations based on clinic needs.
- Serves as a primary care provider for a panel of patients.
- Organizes and manages the care of primary care patients
- Provides routine health maintenance by performing interval well visits
- Complete history and physical, developmental assessment, risk assessment, cancer screening
- Make appropriate referrals for additional services required, including nutrition, social services, behavioral health, and specialty care.
- Participates in an on-call system
- Updates Electronic Medical Records (EMR) appropriately using proper coding and document care
Requirements:
- Completed accredited Nurse Practitioner program in Family Practice (highly preferred) or Adult Medicine
- Current MA NP License, Family Practitioner, Board Certified, Current DEA, MCSR, and Basic Life Support for Healthcare Providers (BLS) required
- Computer Literate (MS Office, Windows), experienced with EMR (EPIC/OCHIN highly desired)
- 3-5 years of NP experience required and bilingual preferred
- Experience with a community health center population and practice setting preferred
