At our South Nampa Clinic, a new model of primary care services is offered where providers will leverage an agile approach that is shaped by consumer preferences and insights. To support and evolve this clinic, the Consumer Access and Experience team seeks a full-time Nurse Practitioner or Physician Assistant to join our dynamic team. This opportunity:provides comprehensive care to pediatric, adult, and senior populations.provides front door care to include wellness, acute, behavioral health, diagnostics, pharmacy, and post-discharge outreach to reduce risk of readmissions.ensures workflows and cross-training is built to minimize transitions during care, while also interfacing with specialists to faciliate shared decision-making.assists with the development, refinement, and enhancement of clinical programs, initiatives, processes, policies, workflows, and projects. Team members can expect collaboration, shared decision making, and partnership with care team members.Minimum Qualifications:Graduation from an accredited school with a minimum of a Master’s Degree and certification, as required by the specialtyEligibility to obtain licensure from State of Idaho to practice as a Nurse Practitioner or Physician Assistant.Must obtain and maintain unrestricted prescriptive authority, DEA, and controlled substance licenses.Eligible to obtain and maintain medical staff membership with SLHS.
