Reason for coverage: Provider moving out of state
Schedule: M - F
Shift: 8am - 5:30p with an hour lunch
Call: no
Patient volume: 18 pts a day
Scope: Ambulatory Primary Care
EMR: NextGen
Hospital: no
Group/support: Receptionist, MA, referrals, and a nurse
Requirements (boards, license): BC - NP/PA with Oregon license
