Job Description: Client is seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our physician-led practice serving Skilled Nursing Facilities (SNFs) throughout the Greater Cleveland, Ohio area.
This role is ideal for an experienced healthcare provider who enjoys providing comprehensive patient care, managing chronic conditions, and collaborating with multidisciplinary teams to improve outcomes for patients in post-acute and long-term care settings.

Job Responsibilities
Provide comprehensive medical care to patients in skilled nursing facilities throughout the Greater Cleveland area.
Perform patient assessments, evaluations, and follow-up visits.
Diagnose and manage acute and chronic medical conditions.
Develop, monitor, and adjust individualized treatment plans.
Manage endocrine-related conditions and support ongoing patient care needs.
Collaborate with facility staff, supervising physicians, nurses, and interdisciplinary care teams.
Review patient progress and coordinate appropriate medical interventions.
Maintain accurate, timely, and compliant clinical documentation.
Ensure high-quality, patient-centered care delivery.
Qualifications
Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the State of Ohio.
Minimum 1 year of endocrinology experience required.
Experience working in:
Skilled Nursing Facilities (SNFs)
Long-Term Care
Geriatric Care
Post-Acute Care settings is preferred.
Strong clinical assessment and decision-making skills.
Excellent communication and interpersonal skills.
Ability to work independently while collaborating effectively with healthcare teams.
Commitment to compassionate, patient-focused care.
Preferred Experience:
Experience managing complex chronic conditions.
Background in endocrinology, internal medicine, geriatrics, or post-acute care.
Familiarity with SNF workflows and interdisciplinary care coordination.
