Our team provides essential primary care services through a network of community clinics and mobile health units, caring for uninsured and Medicaid populations. Position Highlights:
- Location: 5116 Richmond Henrico Turnpike, Richmond VA 23227 and surrounding area (within 1 hour)
- Schedule: Monday – Friday plus 1 Saturdays per month.
- Provide primary care services to children and adults through both the clinic and Mobile Care RV, bringing healthcare directly to underserved communities throughout the Richmond region and surrounding areas within a one-hour radius.
- Partner with City Schools providing sports physicals, immunizations, health screenings, community events, etc.
- Shared call schedule, with RN first call
- Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients’ health holistically.
- Review quality and outcome metrics monthly with the healthcare team
- Collaborate with local civic and faith-based leaders on community education programs
- Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice
- 1+ year APP experience required
- The ability to speak Spanish is extremely helpful in this role, but not required
