*Location: Ontario and Fontana, California
*Start Date: ASAP, as soon as credentialed
*End Date: Ongoing, 3 month commitment requested, with options to extend
*Shift Schedule: Choose between 7a-5p, 3p-11p, 11p-7a, days are Monday through Sunday
*No Call
*Fontana has 3 Ors and Ontario has 2 ORS; 1 OR in each location is open weekends
*Care Team Model; Supervision ratio is 1 MD to 3 CRNAs - 70 total Anesthesia providers
*Case Types: Bread & Butter, GS, Ortho, GYN Ophth, Urology, Neuro, Peds (ages 1-18),Thoracic, L7D/C-Section; May consider providers that can’t do Thoracic, Neuro, Peds, L&D/C-Section.
*EMR: Epic
*Requirements:
- Board Certified CRNAs
- Active, unrestricted CA License
- Active, unrestricted CA DEA License
- Clean Malpractice/NPDB
- ACLS, BLS, MSQA, TEE, BLS, ACLS, PALS, etc. Certifications
- 3 month commitment; highly prefer full time; may consider 2 weeks per month
- Must be able to travel to the 5 different facilities; can be assigned to any of the facilities on demand
*Housing and Travel can be negotiated within the rate.
*Malpractice Insurance can also be negotiated within the rate.
