As a key member of our wound care team, you will:
• Conduct thorough assessments and develop personalized treatment plans that address underlying health
issues and co-morbidities
• Deliver hands-on, bedside wound care to patients in skilled nursing and assisted living facilities
• Lead the charge on wound assessment, treatment planning, and advanced procedures
• Harness cutting-edge tech and evidence-based protocols to accelerate healing
• Guide smart, effective decisions based on a holistic approach in collaboration with facility teams and
families
• Keep your charting sharp and timely in our customized EMR
• Help drive quality initiatives and support our mission of raising the bar in post-acute care
Qualifications:
• MD or DO with active and unrestricted IL license
• Board Certification in Internal Medicine, Family Medicine, Surgery, or similar
• 2 years of physician experience
• At least 2 years of wound care experience
• Previous experience in SNFs, long-term care, or rehabilitation facilities
• IL prescriptive authority & DEA registrations
• Ability to travel daily to assigned facilities in Chicago
• Comfortable with EMR, mobile, and laptop documentation
• Strong interpersonal skills — collaborative, communicative, and adaptable
