Requirements:
- Board Certified in Urology (MD/DO)
- State License: Active Illinois License – Required
- Certifications: BLS & ACLS
- DEA / Controlled Substance: Required
Additional Requirements:
- Must provide vaccination records
- Negative drug screening required
- Physical exam within 90 days prior to start
- 2 professional references required at time of submission
Travel & Logistics:
- Travel and lodging covered per guidelines
- Mileage reimbursement for local candidates

Compensation Notes:
- Orientation: Daily rate (typically prior weekday)
- Ongoing: Hourly or daily rate (M–F schedule)
