• Oct 16, 8am - Oct 19, 8am or Oct 23, 8am - Oct 26, 8am or Oct 30, 8am - Nov 2, 8am I am needing 2 of these weekends filled, the one that isn't filled will be covered by internal provider

Required Procedures: cystoscopy, cystoscopy and dilation

Support Staff: 3 RN's, 1 CNA

Reason for Coverage: Assist permanent provider with call and clinic

Credentialing Information:

Timeframe: 90 days

Requirements: completed credentialing application, ND license, DEA license, supplemental documentation, case logs

Submission Requirements

• Board Certified Urology - Preferred. Will accept Board Eligible providers if they are within 5 years from residency

• Case logs will be requested for reviewal at presentation and offer.

• North Dakota License - Highly Preferred

• Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
