Onsite Urology Physician Services to support the Fargo VA Health Care System.
Period of Performance: 3/17/2026 to 3/16/2027 (Estimated) plus, two 1-year option periods. Effective Date: Date of first Urology Physician shift or 30 days from the Award Date (see Block 31c); whichever occurs sooner.

Qualifications/Candidate Submittals Must Include:

- Proof the provider has successful completion of an approved residency program in Urology during which 3 years were devoted to Urological surgery
- Proof of Board Certification/Eligibility by the American Board of Urology (ABU)
- Unrestricted License
- BLS Certification
- ACLS Certification
- DEA License
- NPI
- OIG Exclusion Search Results
Occupational Health Testing/Immunizations: (Not required for proposal submission)
- MMR vaccine or titer
- Hep B vaccine or titer
- Varicella vaccine or titer
- TB skin test, bloodwork, or Chest X-ray
- Flu shot

VA Medical Center, Fargo, ND is a twenty-four (24) hour medical facility, with designated Urology clinic. Clinic hours are Monday-Friday, 730am-4pm local time
