Practice Setting

• Medical Setting is a Mix of Inpatient & Outpatient responsibilities.
• Patient Volume is approximately 25-30 patients with a 10-bed department.
• Excellent Support Staff, including two MAs and a highly experienced APP.

Schedule

• Ongoing Need with 5-10 shifts per month still available.
• Block Scheduling with Only 1-2 Weeks per Month - you pick the dates that work for you.
• Scheduled time is Monday-Friday (2 days in clinic/3 days in OR).

Procedures and Cases

• Types of Cases are primarily General Adult Urology.
• Procedures details available upon request.
• Patient Ages are exclusively Adult.

Requirements

• Must be Board Certified in Urology.
• Required Certifications include ACLS and BLS.
• Required State Licensure in Arizona (or eligibility for expedited).

Credentialing Requirements

• Credentialing Timeframe allows for Expedited Privileges.
• Will Wait for License if you hold an Active IMLC LOQ.
