• Providers will also need to have or being working towards obtaining the CA FLURO
• The Following Certifications Are Required: ACLS
• Estimated Credentialing Timeframe: 90 days

• Robotic surgery (including partial nephrectomy, intraoperative ultrasound)
• Incontinence procedures
• Penile prosthesis
• Extracorporeal Shock Wave Lithotripsy (ESWL) Additional Information
• Must supervise Advanced Practice Providers
• No residents or teaching required
• Facility will accept one provider only
• CA licensure and fluoroscopy certification support available
