Schedule/Availability Requirements:
- April 25 @ 5p through April 28 @ 7a
- May 23 @ 5p through May 27 @ 7a
- June 06 @ 5p through June 09 @ 7a
Facility Location: Good Samaritan
Setting: Emergency Dept./Surgery
FTE: 1
Duties: Cover Urology call - get handoff from Urologists if appropriate
Required Procedures: General Urology
EMR: EPIC
Support Staff: Surgery nurses and techs. on call
Reason for Coverage: previous locum scheduled cancelled stint
Deal breakers (do not present this): Not willing to learn EPIC
Submission Requirements
- Board Certified/Board Eligible (Urology) - REQUIRED, NOTE AT NAME CLEAR.
- Active NE license - HIGHLY PREFERRED
- Clean Malpractice - HIGHLY PREFERRED
- Availability of dates in the job description - REQUIRED, NOTE AT NAME CLEAR.
- Case logs are required - REQUIRED, INCLUDE AT PRESENTATION.
- Disclose if the provider has worked at any other CHI/Dignity facility - REQUIRED, NOTE AT NAME CLEAR.
- COVID VACCINATED - REQUIRED, NOTE AT NAME CLEAR.
