UROLOGY LOCUMS ILLINOIS
Start date as soon as credentialed.
Immediate Dates needed: June 19-23
June 26-30 and beyond
Board Certified in Urology. BLS.
LICENSE RESTRICTION: We are only considering candidates that are ALREADY ACTIVELY licensed in Illinois or those who already possess the IMLC certification with their participating compact state
SCHEDULE: Full time, weekday clinic--Monday-Friday 8a-5p, one half-day of OR each week, with 24-hour call Q5, and Weekend 24- hour call (5p-7a Friday-Monday). Ideally looking for 1-2 weeks per month.
PROOF OF COVID VACCINATION IS REQUIRED.
*Charting time after hours cannot be billed extra and needs to be part of the base rates provided.
SUBMISSION REQUIREMENTS
BC in Urology, MD or DO
IL License Active – Required OR IMLC
BLS Certification - Required
Availability per request - Required
References per client process – Required
PROOF OF COVID VACCINE IS REQUIRED TO WORK AT THE FACILITY - Required
