About You
- Graduate of Accredited medical school with a degree of Doctor of Medicine (MD) or Doctor of Osteopathy (DO).
- Completion of Accredited Residency
- Board Certified or Board Eligible
- Have or have the ability to obtain an unrestricted license to practice within Georgia
- Current DEA certificate without restriction
