Dates of Coverage: Dec 22 - Jan 5. Potential for ongoing sporadic coverage.
Average Number of Cases Per Day: 2 hearts/week & 2-3 lungs/week = 5-6 procedures/week
Annual ER Volume: 45K/year
Number of Operating Rooms: 16 rooms
Documentation System/EMR: EPIC
Description of Ancillary Staff: CT-NP, CT-PA, CT rounding nurse
Patient Age Demographic: Adult & Geriatric
Estimated Credentialing Timeframe: 30 days
Scope of Services Required:
- Adult Cardiac Surgery
- General Thoracic Surgery
Coronary Artery Bypass Surgery Procedures Required:
- CABG
- Arterial Revascularization (Using Internal Mammary Arteries, Radial Artery and Gastroepiploic Artery)
- Redo Coronary Revascularization (With or Without Heart Valve Surgery)
Procedures Required:
- Tube Thoracostomy
- Pericardial Window
Thoracic Surgery Procedures Required:
- Thoracotomy Lung Biopsy
- Thoracotomy Lung Resections and Pneumonectomy
- Thoracotomy Decortication
- Chest Wall Resection and Reconstruction
- Mediastinal Surgery
- Mediastinotomy and Biopsy
Additional Surgeries Required:
- Heart Valve Repair/Replacement Surgery
- Heart Valve Replacement Without Surgery (CoreValve® System)
- Mitral Valve Repair
- Board Certification or Board Eligible - [REQUIRED]
- Active IN License at the time of submission - [REQUIRED]
- Active IN DEA/CSR at time of submission - [REQUIRED]
- Conversion Fee of 25k - [REQUIRED]
- 24-hr call must include minimum of 4 hours gratis - [REQUIRED]
