Requirements

Board Certification: Must be BC or BE in Neurosurgery

Licensure: Active Arizona license preferred; IMLC candidates considered

DEA: Required, AZ registration

CSR: Not required
-
Certifications: BLS required
-
Experience: Trauma experience required; busy Level I Trauma background preferred

Expedited Privileges: May be possible with clean file
