Physician - Neurological Surgery

• We are still in need of coverage for the below dates. Please bid providers interested ASAP! * We can look to pair together coverage if needed!

SCHEDULE

• Coverage Type: Inpatient and Outpatient
• Schedule Type: Clinic and Call
• Call Type: Weeknight Beeper and 24 Hour Weekend Beeper Call

We would like to note they may not be in clinic all 5 days during the dates listed but it is a possibility.

• Dates of Need: See above
• Shift Times: 8:30am-5pm , 5pm-8:30am
• Lunch N/A

Please ensure bids reflect the following:

• Guaranteed Hours per Day: 8
• Hours Worked for OT to apply: 9 hours per day
• Rates to be Filled Out on Bid: Clinic and; Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back.

CREDENTIALING

• Current CV (must be uploaded to bid)
• License: Active
• DEA Required: Yes
• Boards: Must be Board Certified or truly Board Eligible within 3-5 years of residency is an option but we are looking at BC.
• Skills/Certs: Will see complex spine cases, No airway certifications required
• Other: FLURO certification is required (they can have the temp FLURO)

We will need providers to submit during the privileging process 100 cases within the past 24 months specific to the privileges being requested.

PLEASE ENSURE AN UPDATED CV WITH ALL HOSPITAL AFFILIATIONS IS UPLOADED DURING PRESENTATION.

Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation.

o All HBS specialties require 2 years of current Hospital clinical activity in the specialty.
o Gaps greater than 3 months must be explained.
o Include current number of case logs upon presentation for surgical specialties.
