**Seeking expedited credentialing so Oregon license MUST be in hand at time of presentation & candidate must be able to get through expedited credentialing (Must have availability complete application and verifications in a timely manner).
Reason for opening: Gap in coverage
Type of privileges (clinic/hospital/both): Both
Setting: Inpatient, Outpatient or Both? Both
If Inpatient, # of beds: 21
Rounding, admissions, or none? Admissions
Patient demographics: Adults
Average patients per day: 10
Required Procedures: General surgeon critical access. Colonoscopy and endoscopy required.
Required clinical activity in the last 24 months (case logs): Applicant must provide documentation of provision of surgical services representative of the scope and complexity of the privileges requested during the previous 24 months (applicants who completed training during the previous year must provide a training log).
Will provider work solo? If not, other providers specialties working with them: No
Licensed Candidate: Yes, given our timeline, Oregon license to be in hand at time of presentation
Required certifications (Must be from American Heart Association or American Red Cross as applicable): ACLS
EMR: EPIC

24 hour call is mostly for ED but they will assist in the clinic as needed for follows up. Average onsite/callback volume will be about 8 hours for each 24-hour call shift.
Current Scheduled Needs: 5-10 days/month Would like to fill with one candidate who can work them all:
April 14 - orientation shift
April 15-20th
May 14-25th
July 9-13th
Sept 2-15th
Sept 28-Oct 13th – This is a gap we have, but we won’t need the provider to take all of these dates. We are open to hearing what the candidate can offer.
