- Date: March 2026 - ongoing
- Schedule: M-F 8a-5p, one weekend of call per month
- Practice Setting: Hospital-based clinic + OR
- Patient Volume: average 20 per day in clinic, 3 cases per OR day
- Requirements: AZ license (will wait for license), BC/BE
- Provider must obtain Medicare/Medicaid enrollment prior to start
- Credentialing: 60-90 days
