JOB OVERVIEW
• Job Title: Surgeon – General Surgery
• Job Type: Permanent – Full-Time
• Location: Hobart and Valparaiso, IN
• Service Setting: Inpatient and Outpatient
• Schedule: Monday – Friday
• On-Call Expectations: 1 in 5 rotation
• Start Date: Approximately 90–120 days post-offer, pending credentialing
PATIENT POPULATION & SCOPE
• Patient Demographics: Adult surgical patients
• Admissions & Rounding: Included
• Consults & Follow-Ups: Yes
• Case Mix / Clinical Focus Areas: General Surgery; subspecialty interests supported
FACILITY & TEAM DETAILS
• EMR System: Epic – HIMSS Stage VII
• Specialty Backup: Collegial physician group support
• Support Staff: Integrated inpatient and outpatient care teams
• Leadership & Reporting Structure: Hospital-employed physician model
• Reason for Opening: Growth and expansion of surgical services
CREDENTIALING & COMPLIANCE
• Board Requirements: BC/BE in General Surgery
• DEA & Certifications: DEA, CSR required
• Credentialing Timeline: 90–120 days

REQUIRED SKILLS & QUALIFICATIONS
• Board Status: BC/BE in General Surgery
• State License Required: Eligibility for Indiana medical license
• Certifications: DEA, CSR
• Experience: Previous general surgery experience preferred
• Additional Requirements: EPIC proficiency, collaborative practice approach
DUTIES & RESPONSIBILITIES
• Perform general surgical procedures in inpatient and outpatient settings
• Participate in call rotation (1 in 5)
• Provide consultative services and follow-up care
• Collaborate with multidisciplinary healthcare teams
• Maintain accurate documentation using EPIC EMR system
