Orthopedic Surgeon Physician needed for Hazen, ND (Locum)
  1. Order Description:
    1. Type: locum
    2. Position: physician
    3. Role: procedures and clinic
    4. Annual volume: over 800 referrals from facility per year
    5. Shift options: clinic hours, no on-call
    6. EMR:

  2. Candidate Requirements:
    1. State License(s) accepted: ND
    2. DEA/CSR required?: Yes
    3. Covid/Flu Vaccination Requirements:
    4. BC/BE – will site accept non-boarded? Under what conditions:
    5. What boards are accepted? ( EM, IM, FP) note if site accepts specialty board (ABPS) or not?
    6. c. Residents –Y/N and what are the details?
    7. Life Certifications needed (ATLS/CALS, ACLS, PALS, BLS, NRP, ALSO, etc.), is ATLS waived if ABEM:
    8. Will facility accept CALS in lieu of ATLS: Y/N
    9. Procedures logs needed or past amount of experience required and in what timeframe ( Ex: 2 years, 500 hrs):
