• Type of Position (Permanent or Locum Tenens):Permanent, Hospital Employed
• Shift/Work Schedule:Typical schedule is 1-1.5 days in the clinic, 3-3.5 days in the OR (.5 days
in wound care if desired)
• Work Hours: Full time (see above)
• Call Schedule (if applicable):Call: 1:3 every night it rotates and every 3rd weekend and holiday
(can switch around because they get their schedule out a year in advance). Must be within 30
mins while on call.
• Start Date: Retirement is at the end of the year so whenever the candidate can start
• Facility Type (Hospital, Clinic, Private Practice): Hospital (clinic is located within the hospital)
• Clinical Setting (Inpatient, Outpatient or Telemedicine): See above
• Patients Per day: 14-16 patients on clinic days, If a surgery day-average cases per day is 8
(including endoscopy)
• Procedures required: We are a bread and butter general practice, no trauma, vascular, or
thoracic cases.

• Base Salary/Total compensation range: $455k total annual compensation (typical break out is
$400,000 base and $55,00 for call)- 2 year guarantee.
• Bonus/RVU, sign on, relocation or Incentive structure: Typically a $50k signing bonus.
Physician can earn a productivity bonus in an amount equal to $69 per wRVU performed by
Physician in excess of 5,800.
Up to $10,000 relocation assistance.

Benefits Offered: (Health, Retirement, CME, Relocation Etc):
● All premiums paid by hospital for employee and family (health/dental/vision) plus we
contribute $1,630 annually to an HSA (if on hospital’s health plan).
● Pension plan for retirement: Will match up to 3% of gross wages. Towards the end of
year, we will pay an additional amount equal to 9.7% of earnings which can be deferred
to a pre-tax 403b, 457b account or an after-tax IRA.
● CME: $5000.
● Subscription and Dues: $1500.
● Cell phone allowance $1080.
● Residency Stipend-We will pay a residency a month stipend if they sign with us.
● We offer loan repayment on a per contract basis.
