SCHEDULE : Mon-Fri 7:30A-4:30P
Practice Setting: Hospital and outpatient locations
Average Number of Cases Per Day: variable
Annual ER Volume: 120 pts day
Documentation System/EMR: Epic/PowerScribe/PACS
Number of Radiologists in Practice: 8 on team -2 leaving =6 total after Feb
Description of Support Staff: Full Compliment
Patient Age Demographic: Adolescent, Child, Adult, Geriatric
Estimated Credentialing Timeframe: 90-120 days with completion on apps
Submission Requirements
- Board Certified or Board Eligible - [REQUIRED]
- Active MA license or pending MA application with submission date and estimated approval at time of name clear. [REQUIRED]
- ACLS, BLS - [REQUIRED]
- A Self-Query NPDB report is required at the time of presentation and must be pulled within the same month as the submission. - [REQUIRED]
