Start Date: December 20, 2024 - Ongoing
Schedule/Shift Needs: Monday - Friday Clinic Hours, 14 days on and 14 days off
Submission Requirements: ***Expedited Credentialing in 2 weeks with CLEAN Record!***
- Board Certified or Board Eligible: REQUIRED
- Active KY License or IMLC, CS and DEA ( or available DEA to transfer )- REQUIRED
- Availability per request - REQUIRED
- Clean Malpractice: REQUIRED
