Group is seeking an experienced Nurse Practitioner to join a cardiothoracic team as a
thoracic nurse navigator. The thoracic Nurse Practitioner will be working with a team of 2 cardiothoracic surgeons and 7 NPs/PAs, Practice Manager, care transformation
nurse, MAs and support staff.

The thoracic NP will have primary responsibilities of inpatient rounding, inpatient medical management, and discharge planning coordination for all postoperative thoracic surgery patients. Additional responsibilities include oversight of the outpatient thoracic clinic including but not limited to referral intake, conducting postoperative physical exams, medication management, care management and coordinating referrals to medical oncology and pulmonology.
