Schedule/Availability Requirements:
Clinic Hours- Mon-Fri 8a-5p
Fulltime clinic coverage
The structure of the day is an outpatient clinic, 8am - 5pm, 5 days per week. There will be 1/2 day to 1 day per week dedicated to procedure time (Bronchs), depending on the need of the individual provider. All other items listed under "procedures" fall under outpatient management within the clinic. PFTs are performed by RTs and read by the providers.
Submission Requirements
ALL OF THE FOLLOWING MUST BE NOTED IN ADDITIONAL SUBMISSION DETAILS AT NAME CLEAR TO BE CONSIDERED AND AVOID BEING REDIRECTED!!
- Board Certified Pulmonology - REQUIRED
- Active WA License - REQUIRED OR IMLC (LOQ MUST BE IN HAND AT TIME OF SUBMISSION)
- Pulmonology Residency/Fellowship - REQUIRED
- Active BLS - REQUIRED (Needs to note willing to obtain if not active)
- Clean Malpractice/Background - HIGHLY PREFERRED\
- Availability at time of name clear – Required – must provide minimum 30 days of availability from providers start date. Providers who provide specific and accurate availability will be considered first. Vague availability will be reviewed last. Black out dates required.
