Will other Physicians provide coverage during locums coverage dates (list names): Yes – Dr. Yadam will be working the clinic at the same time

Describe support staff: RN, Medical Assistant, and Patient Support Associate

Dictation system: Dragon

Documentation system/EMR: Epic

Do you use Teleradiology? Yes, after hours and weekends, only when needed

Medical school MUST be on the California Approved Medical School List

Residency and Fellowship MUST be ACGME Approved

Minimum 25 patient logs within 12 months

Patients per shift: 15
