Job Overview – Physician (SNF, LTC, ALC):
Compensation: $350,000 – $400,000/year

Schedule: Monday to Friday, 9:00 AM to 3:00 PM
Join our client as a Physician (SNF, LTC, ALC) in Torrington, CT, providing primary and transitional care to adult and geriatric patients in skilled nursing facilities, long-term care, and assisted living communities. This flexible weekday role is ideal for Internal or Family Medicine physicians with experience in geriatrics, hospitalist care, or post-acute settings. You’ll manage chronic conditions, collaborate with interdisciplinary teams, and help deliver high-quality, continuity-based care across multiple sites.

Responsibilities as the Physician:
- Patient Care Management: Provide high-quality primary and chronic care to adult and geriatric patients in SNF, LTC, and ALC settings.
- Clinical Oversight: Monitor and adjust treatment plans, perform in-person assessments, and ensure continuity of care across multiple facilities.
- Interdisciplinary Coordination: Collaborate with nursing staff, specialists, and family members to ensure cohesive and patient-centered care.
- Documentation & Compliance: Maintain timely and accurate EHR documentation in accordance with state and federal healthcare guidelines.
- Facility Rounds: Travel to multiple partner facilities across Connecticut to provide hands-on care, clinical oversight, and patient follow-ups.
- Licensure: Active and unrestricted Physician’s Medical License in the state of Connecticut required.
- Certification: DEA license and Connecticut Controlled Substance Registration (CSR) required.
- Experience: 3+ years of experience in skilled nursing, hospitalist, hospice, or geriatric care.
- Technical Skills: Proficiency in EHR systems (e.g., Epic, eClinicalWorks) and digital charting in post-acute care settings.
- Skills & Attributes: Compassionate and patient-focused with strong clinical judgment, adaptability, and the ability to work independently across multiple care environments.
