Provides comprehensive psychiatric care including: evaluations, screenings, consultations and medication management and follow-up for complex behavioral health patients across care settings.
Requirements:
- Completion of accredited medical college/university with Doctor of Medicine (MD) or Doctor of Osteopathy (DO)
- Successful completion of internship residency and, if applicable, fellowship
- Current board certification/eligibility in specialty
- If newly graduated from residency, certification must be completed within three years of graduation
- Unrestricted license to practice in New Hampshire
- Current DEA certificate without restriction
- CPR with AED within 6 months of hire
- High degree of independence, accountability, flexibility and initiative required
- Motivational interviewing experience preferred
- Proficient in Microsoft Office and EMR documentation
Responsibilities:
- Performs comprehensive psychiatric care including, but not limited to, face-to-face evaluations, consultation, follow-up, diagnosis and medication management and discharge recommendations across primary care, emergency department and inpatient settings.
- Be available to primary care for ad hoc telephone or electronic consultations, and manage a small outpatient panel of high-risk individuals
- Provider regularly scheduled (weekly) caseload consultations with assigned care coordinators in primary care. This includes oversight of clinical outcomes, suggestion of treatment plan changes, medication recommendations and discussion of referrals.
- Provide coverage to the Exeter Hospital Emergency Department and on the inpatient units for patients on mental health holds. This will include treatment planning, medication management and participation in discharge planning activities.
- Provides leadership to, and works collaboratively as part of the integrated behavioral health team across the care continuum affiliates (ambulatory, emergency and inpatient).
- Utilizes and assists in developing evidence-based standards, guidelines and protocols for psychiatric care delivery. Incorporates data and information to continuously improve care practice to enhance patient outcomes. Participates in quality improvement projects and initiatives, monitors relevant research and best-practice to assist in the revision of above guidelines, as appropriate.
- Provides teaching to patients and families to influence patient outcomes, self-management behaviors and adjustment to illness. Identifies learning needs of various populations, including clinical staff, and contributes to the development of education programs to meet those needs.
- Fulfills specialty specific professional responsibilities.
- Documents all patient care activities accurately and in a timely manner as per policy and procedure and according to regulatory mandates.
- Performs other duties as required or assigned.
