Provider Requirements:

• Provider Type: Physician (MD/DO)
• Provider Credentials: Board Certified or Board Eligible in Psychiatry
• Board Status: Board Certified or Board Eligible
• Certifications Needed: Must hold or be eligible for an Indiana medical license
• Willing to License: No – active Indiana license required

Practice Details:

• Setting: 100% Outpatient
• Schedule: Monday – Friday, daytime hours (exact schedule to be discussed during interview)
• Call: No call required
• Patient Population: Primarily adult patients
• Case Mix: General outpatient psychiatric care
• Procedures: Standard outpatient psychiatric evaluations and medication management
