PATIENT INFORMATION
  • Patient Demographics: Ages 5+; Developmentally disabled patients with mental health disorders
  • Patient Volume/Census: 610 clients per 7.5-hour day depending on visit type

  • Phone Consults: Yes, via telehealth as needed
  • Case Mix: Anxiety Disorder, Intermittent Explosive Disorder, Depression, Disruptive Mood Disorder, OCD, Schizoaffective Disorder, Bipolar Disorder; small fraction involves JV220 forms

JOB REQUIREMENTS
  • Licenses: Active CA State License
  • Board: Board Certified or Board Eligible (Certification required within 5 years of residency)
  • DEA: Required; authorized to prescribe controlled substances
  • Certifications: Flu vaccination; COVID vaccination or approved exemption
  • Experience: Ability to treat developmental disability + mental health population
  • Other Qualifications: Must enter hours in Vemsta platform; strong communication, documentation, and interpersonal skills; ability to work with diverse populations

DUTIES & RESPONSIBILITIES
  • Perform psychiatric assessments, evaluations, and MSEs, providing DSM-based diagnoses
  • Provide ongoing medication management for assigned patients
  • Use the clinic's electronic health record system and follow Santa Clara County Psychiatric Medical Guidelines
  • Collaborate with medical director, nurses, clinicians, and case managers for coordinated care
  • Obtain and review diagnostic data, labs, and consultations relevant to patient treatment
  • Prepare detailed written reports, documentation, diagnoses, and summaries as required
  • Contribute to individualized treatment planning with clinical staff
  • Provide psychiatric services in outpatient clinics via telehealth and in person
  • Offer consultation and peer guidance to psychiatrists and therapists as needed
  • Consult with clinic leadership regarding policies and treatment-related issues
  • Participate in quality assurance, quality improvement, and performance improvement activities
  • Advise families and caregivers regarding diagnoses, treatment, and intersection with developmental disabilities
  • Prepare legal and court-related documents as necessary
