- Part Time
- Hours: 20 - 32 hours per week; 11 – 8 one night p/wk and 9 – 4 one Saturday per month

- Average Patients seen: 1.5 per hour

- Sign-On Bonus: $10000
- Compensation: - $120 to $160/hr - $10,000 Sign-on Bonus & Relocation package available (Will be prorated for PT) - Productivity Bonus as well (Will be prorated for PT)

Initial assessment and patients will then be passed onto the LCSW team, next visits will be for follow-up and prescription refills. Truly integrated healthcare approach QUALIFICATIONS: - CA license REQUIRED - 1-year experience in an ambulatory care setting REQUIRED - Experience in FQHC PREFERRED - Bilingual (SPAN-ENG) PREFERRED
