NYC Health + Hospitals/Kings County, located in the heart of Brooklyn, accommodate more than 518,076 outpatient visits, more than 141,328 emergency room visits, and with 627 beds, more than 25,000 inpatient admissions annually. The hospital maintains a strong academic affiliation with SUNY Downstate Medical Center in order to maintain its high standards of healthcare delivery.
Kings County is located in the East Flatbush/Prospect-Lefferts section of Brooklyn, close to the 2 and 5 subways. Our patients are diverse, with the majority being afro-Caribbean. Most of our patients are native English speakers. Knowledge of French/Haitian Creole is helpful, but not required.
Responsibilities:
Kings County’s uniquely designed 25- bed subacute inpatient unit will serve individuals with severe mental illness that require more comprehensive treatment in order to be able to maintain stability outside of the hospital. During patient’s 4-month stay, individuals are treated through a social learning model, which, in addition to medication management and psychotherapy, highlights the role of community-based learning (groups) and behavioral reinforcement (rewards for prosocial behavior) to maneuver through challenges, stressors and other potentially anxiety provoking situations. Many individuals return to the hospital after discharge because they have not learned how to engage in what we identify as the social determinants of health leading to isolation, nonadherence and psychiatric decompensation. The ECU puts a greater focus on teaching patients with severe mental illness to engage successfully with others through incentivized learning. The multidisciplinary treatment team is comprised of nursing staff, social workers, creative arts therapists, occupational therapist, housing specialist, rehab counselor, data manager, peer counselor, behavioral health associates, as well as psychologist, nurse practitioners, physician and patient. Together, this team is responsible for comprehensive care during hospitalization in this sub-acute unit and for coordination of individualized, strengths-based aftercare planning for community success.
We are seeking a dedicated, passionate, and team-oriented physician to partner with our program director to lead the interdisciplinary care. Physician will carry a clinical caseload and oversee work conducted by psychiatric nurse practitioners and other disciplines where appropriate. This specialty service will provide both significant meaning to any professional’s life as well as to truly serve the community need.
Oversee relevant Extended care unit inpatient service functioning including, but not limited to:
- Policies and procedures regulating delivery of patient care on the service
- Coordination of care between disciplines and functioning of interdisciplinary treatment teams.
- Development and provision of treatment in cooperation with other members of multidisciplinary treatment team members.
- Establishing, monitoring and maintaining professional standards of work in collaboration with other disciplines coordination of professional staff schedules to ensure adequate coverage at all times.
- Monitoring workload and work assignments and effecting staff and program changes to ensure highest quality of patient care.
- Liaises with other departments and services, including but not limited to other inpatients in BH, and transfer from other facilities.
- Liaises with Central Office’s Office of BH, DOH, and OMH.
- Ensure timely application of ACT/AOT/IMT/State application and person-centered psychiatric evaluations for housing applications.
Quality assurance, performance improvement and regulatory affairs:
- Maintains up-to-date knowledge of standards for certification by regulatory agencies.
- Participates in relevant PI & QA activities and initiatives, both locally and in collaboration with OBH/OMH
- Maintains awareness of performance improvement indicator and leads interventions and initiatives to meet goals for the service.
- Prepares annual report for the service.
- In collaboration with risk management reviews major incidents on the service and leads and monitors relevant corrective action when needed.
- Works with all staff to ensure timely and adequate medical record keeping and monitors the quality of documentation.
- Provides clinical consultation and supervision as needed to all staff on the acute inpatient service.
- Conducts and coordinates tier reviews as per policy
- Maintains focus on key strategic priorities including patient experience, access to care, increased market share, and improving financial stability
Education and supervision:
- Maintain an updated and comprehensive set of psychiatric practice guidelines including administration of medication in accordance with generally accepted professional standards that address the indication, contradictions screening and monitoring procedures, dose requirements and expected outcomes.
- Ensure and monitor that the pharmacological components of treatment plan reflect the exercise of professional judgement for the medication treatment including diagnosis, target symptoms, risk and benefits of particular medication, and consideration of alternate treatments.
- Ensure that the rationale for each patient’s course of treatment is documented by the physician
- Include all above in standards of work expected of Attending and Nurse Practitioner and monitor and reinforce said standards of work;
- Teach and supervise psychiatry trainees and medical students
Qualifications:
- Must be board eligible or board certified in Adult Psychiatry
- M.D. or D.O. from an accredited school of medicine
- Completed residency training in an accredited program within Psychiatry
- License to practice medicine in the State of New York
- At least two years of experience in leadership roles
- Proof of Covid-19 vaccination is required prior to hire
