Job Responsibilities
• Develop and implement preventive care strategies and wellness programs 🌿
• Conduct health risk assessments and screenings 🩺
• Manage chronic disease prevention and health promotion initiatives
• Provide patient education on lifestyle modifications and disease prevention
• Collaborate with multidisciplinary teams and community resources 🤝
• Maintain accurate and timely documentation
Qualifications
• MD/DO with board certification/eligibility in Preventive Medicine or related field
• Active and unrestricted medical license
• Strong clinical and analytical skills
• Passion for population health and preventive care
