JOB REQUIREMENTS
- Licenses: Active VT license (or ability to obtain)
- Board: Board Certified in Physiatry preferred (Neurology, Pain Medicine and Anesthesiology)
- DEA: Active DEA Certification
- Certifications: BLS
- Experience: Experience with botox for spasticity management preferred
- Other Qualifications: Ability to practice independently
DUTIES & RESPONSIBILITIES
- Provide outpatient spasticity management
- Perform botox injections for appropriate patient cases
- Maintain clinic workflow two days per week
- Collaborate with existing physiatry program
- Document accurately in Epic EMR
