- Scope of Services: Full Primary & Family Care, Peds, BH, Case Management, Womens Health including Pre & Post-Natal, Peds, On-site Lab, Chronic Disease Care/Mgmt, Preventative, Lab, Health Education, Immunizations, Transportation, Enrollment, Referrals and Specialty Services
- Compensation: $260,000 - $300,000 Salary (DOE) + Sign-on Bonus.

- Schedule: Monday - Friday, 8:30am - 5:30pm. No weekends, No inpatient duties, limited phone-call rotation
Job Responsibilities
- Performs physical examinations of new and existing patients and orders the diagnostic tests required to baseline relevant values.
- Interprets examination findings and test results to develop and implement appropriate treatment plans with patients and other care team members.
- Prepares and reviews patient case histories, interviewing patients and family members to diagnose presenting problems
- Facilitates health and disease patient education, including leading group office visits and annual preventive visits. Supports patient self-management of disease and behavior modification interventions.
- Determines need for consultation or referral and assists in medical care treatment provided at the direction of other specialists. As indicated, refers patients to affiliated subspecialists or for inpatient treatment.
- Collaborates with a multilevel, multidisciplinary team of clinical and non-clinical staff to provide outpatient services as scheduled, following appropriate quality and productivity standards of care.
- Collaborates with Behavioral Health and Wellness colleagues to integrate health, behavioral health, and wellness care for all patients.
- Participates in clinical quality improvement processes, peer reviews and certifications.
- Performs charting and utilizes Electronic Health Records
- Utilizes standing orders and other generally accepted care practices to enable other team members to practice to the top of their licenses.
- As appropriate, supervises and teaches mid-level providers, residents, and medical students.
- Participates in team meetings, data collection, health outcomes reporting, clinical audits, program evaluation, and quality improvement.
- On a rotational basis, serves as an on-call physician for afterhours support.
- Provides coverage for other clinicians when needed
- Adheres to established internal policies, procedures and objectives
- May participate in outside events such as health fairs and other community health related activities.
Qualifications
- Maintain current, valid, unrestricted professional license to practice in the state of California
- Active and unrestricted DEA License
- Knowledge of the principles and practices of family medicine, general internal medicine, or pediatrics.
- Knowledge of the local, state, and federal regulations pertaining to medicine and to community health centers.
- High level of written skills
- Ability to communicate well with people of diverse cultural professional and experiential backgrounds
- Ability to establish and maintain effective work relationships as part of a multi-disciplinary team
- English language proficiency required. Proficiency in another language (Mandarin, Cantonese, Spanish or Vietnamese) preferred.
