- Clinical Role:

- Practice Setting - Office/Clinic
- Dates of Coverage: 8/15/22-8/19/22, 8/22/22-8/26/22 (10 total weekdays)
- No weekend or call coverage needed
- Schedule (Shifts and Hours): 9 am- 5pm, Mon-Fri
- Average Number of Well Check Visits Per Day: 5-8
- Number of Providers in Practice: 1MD/DO & 1 APP
- Responsible for Supervising APPs
- Support Staff: medical assistant, front office staff, manager, pediatric nurse practitioner.
- Average Number of Patients Per Day: 25 per provider
- Nursery Level (I, II, III): 1
- Number of Nursery Beds: 5
- EMR: Cerner community works
- Trauma Level/Designation: None
- BC/BE - Required
- Local, drive-in providers - Highly Preferred
- MI license & CSR – Required
