The Opportunity

This is a high-volume, outpatient PM&R opportunity within a well-established pain management practice serving a dense, underserved population. The role is designed to replace a departing physician’s panel, offering immediate access to patient volume with the ability to rebuild and expand a thriving non-interventional program. The practice operates within a modern, team-based model supported by nurse practitioners, advanced diagnostics, and on-site procedural capabilities.

What makes this opportunity unique is the balance of true PM&R medicine (MSK + EMG) with strong infrastructure and volume already in place. Physicians are supported by a highly efficient clinical team, access to advanced imaging and an ASC, and a predictable weekday schedule with no call responsibilities. There is also optional exposure to procedural work for those interested, along with long-term upside tied to collections and ASC participation.

Lifestyle

  • Call: None
  • Schedule: Monday–Friday
    • 9:00 AM – 5:00 PM OR 8:00 AM – 4:00 PM
  • No 4-day workweek option (5 days required)

Time Off:

  • PTO: 20 days (after 90 days)
  • CME: 5 days + $5,000

Work Environment:

  • Outpatient only
  • Structured schedule
  • Predictable daily flow

After-Hours:

  • No call
  • Minimal after-hours responsibilities

Flexibility:

  • Limited schedule flexibility (must be in-clinic 5 days/week)
  • Optional future opportunity for 1 day/week in satellite location

Compensation

  • Base Salary: $350,000 (guaranteed, 2-year contract)
  • Productivity: Collections-based upside after year 2
  • Realistic Total Compensation: $700,000 – $800,000

Additional Compensation:

  • Signing Bonus: $10,000 – $15,000
  • Relocation: $10,000

Partnership Track:

  • Eligible after 2 years
  • Based on ~$700K collections threshold
  • ASC ownership available (if participating in procedures)

Ancillary Upside:

  • ASC participation
  • Procedure volume
  • High patient throughput

Job Responsibilities

Volume Expectations:

~20 patients/day for incoming physician
~60% new patients
20–30% procedural conversion across practice

Procedural Access (Optional):
ASC (onsite) access with block-style scheduling
1 sedation day + 1 local injection day
20–30 cases/day when utilized

Qualifications

Board Certified or Board Eligible in PM&R
MD or DO
Comfortable with:
MSK
EMG/electrodiagnostics
Outpatient pain management
Ability to work in high-volume, urban setting
Strong communication and teamwork skills
Must be able to obtain state license
No visa sponsorship available

Case Mix:

High percentage of accident-based and government payor patients
Complex pain cases requiring longer visits

Referral Sources:

Established internal volume + walk-in model
NP-driven patient funnel
Built-in procedural conversion (20–30%)

Clinic vs Procedures:

4 days clinic
1 day procedures (optional/expandable)

Sample Weekly Schedule:

Monday–Thursday: Clinic (new + follow-ups + EMG)
Friday: Optional procedure day or clinic

Growth Plan:

Rebuild and expand existing panel
Opportunity to increase procedural exposure if desired
Long-term scalability through volume + collections
