- Monday - Thursday 7:30am - 5:00pm, Friday 7:30am - 12:00pm
- Clinic Setting
- Required Procedures: Spinal Cord Stimulator experience preferred but not required, Epidural Leads
- Credentialing Timeframe: 90+ day
