Location: HWY 264 MP 388, Polacca, Arizona 86042
Contract Type: Federal One-Year Contract (Base Period) — Indian Health Service BPA Call, nonpersonal health care services
Estimated Start Date: November 1, 2026 (facility estimate; subject to award, security clearance, and credentialing/privileging timelines)
Schedule: Full-Time: Monday–Friday, 0800–1700 (8-hr shifts, 1-hr unpaid lunch, 40 hrs/week paid). Part-Time/Intermittent blocks also available (AM 0800–1200 / PM 1300–1700, Mon–Thu, plus Fri AM). FLSA overtime applies over 80 hrs in a biweekly HHS pay period, priced separately.
Credentialing Lead Time: Candidate identified at least 45 calendar days before the scheduled performance date for IHS security clearance and credentialing/privileging per IHM Part 3, Chapter 23. Required documents: resume, OF-306 + Addendum (wet signature), IHS Provisional Access Memo, Pro-Children Act/No FEAR Act/ISSA training certificates, OFI-86C with fingerprint cards, HHS-745 Section A, two valid IDs, education/licensure, BLS certification.
Key Responsibilities
- Deliver full-scope, AAAHC-consistent primary eye care: comprehensive exams, accurate diagnosis, and treatment of ocular conditions.
- Prescribe corrective lenses/medications within licensure and IHS-granted privileges; perform minor ocular procedures per AAAHC Ch. 10.
- Document all encounters in EHR/RPMS the same day, immediately following the encounter (AAAHC Ch. 6 / IHM Pt. 6, Ch. 3).
- Complete timely, accurate referrals (including Purchase Referred Care) for care beyond primary eye-care scope.
Minimum Qualifications

Job Responsibilities
Evaluate, diagnose, treat, and manage refractive error, ocular disease, and related eye conditions within scope of practice
Prescribe corrective lenses, medications, and other vision-related treatment plans when clinically indicated
Perform or interpret diagnostic testing and minor ocular procedures within scope of practice and local clinical protocol
Document patient encounters, prescriptions, procedures, diagnostic results, and disposition same-day in the electronic health record (EHR)
Coordinate referrals and consultations to ophthalmology, emergency care, or other specialty care when a patient's needs exceed clinic capability or granted privileges
Support timely patient access and continuity of care during scheduled clinic sessions while exercising independent clinical judgment
Maintain compliance with infection prevention and instrument sanitation/reprocessing requirements
Uphold patient rights, informed consent, privacy, and culturally responsive care in all encounters
Participate in required orientation, credentialing, peer review, and quality assurance activities
Maintain current licensure, prescribing authority, BLS certification (if required locally), and facility credentialing/privileging throughout the assignment
Qualifications
Active, unrestricted optometry license in any U.S. State, D.C., Puerto Rico, or a U.S. Territory
Minimum two (2) years of recent clinical experience in primary eye care, ambulatory optometry, or a closely related setting
Current Basic Life Support (BLS) certification from an AHA- or ARC-approved provider (required if local privileging standards call for it)
Demonstrated ability to document care in an electronic health record system
Any registration, certification, or authority required to prescribe medications, order diagnostic testing, or perform privileged procedures under local policy
Ability to obtain and maintain security access, credentialing, and clinical privileges at the assigned facility
Compliance with continuing education, immunization, and mandatory training requirements
Working Hours
Part-Time/Intermittent: 4-hour half-days up to 4 full days per week — AM block (8:00–12:00) and PM block (1:00–5:00), Monday through Thursday, plus Friday AM only. Scheduling coordinated by HHCC. Shifts may be cancelled or shortened without penalty before arrival; unpaid lunch periods are not compensated. Overtime is priced separately and follows FLSA rules for hours over 80 in a biweekly HHS pay period.
