- Procedures: Comprehensive clinical ophthalmology procedures, including the full range of oculoplastic and glaucoma services.

Requirements:
- Active unrestricted license in any State or U.S. territory
- Board Certified/Board Eligible by the American Board of Ophthalmology (ABO)

- NPDB Report Please note: Physicians who have ever had a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.
