Practice Setting

Patient population: Adult and geriatric patients

Average patients per day: 12–16

Provider will work solo
Clinical Details

Provide medical oncology and hematology services
-
Manage outpatient oncology patients receiving infusion therapies
-
No required procedures
Documentation Requirements
-
Applicant must provide documentation showing provision of hematology/oncology services within the last 24 months
-
Case logs must demonstrate scope and complexity consistent with requested privileges
Clinical Support and Technology

EMR: EPIC (proficient user required)
Requirements

Board certified Medical Oncology
-
Active Alaska medical license required at time of presentation

Active Alaska DEA required
-
BLS certification required
